Frequently Asked Questions

  • For your first time to our office, we ask that you arrive at least 30 minutes prior to your scheduled appointment in order to process your paperwork. Click here to download our new patient forms.

  • Please bring with you the following:

    – Valid drivers license
    – Current copy of insurance card
    – All medications you are currently on including over the counter medications
    – Medical records from previous primary care physician

  • We accept cash, checks, and major credit cards (Visa, Mastercard, and Discover). Please note, we do not accept checks on the first visit.

  • Yes, we offer same day appointments for our established patients. Please call the office at 407-332-6366 to schedule your appointment.

  • Our office hours are

    Monday through Friday, 8:00 a.m. to 5:00 p.m. Dr. Krupitsky is available Monday through Thursday evening for after hour emergencies. We are involved with a physician weekend call group. There will always be a physician available by phone.

    Please limit weekend calls to acute illnesses or emergencies only. Prescription refills are only available during normal weekday hours since the weekend call group does not refill routine prescriptions or narcotic medications.
